In reality Japanese food portions are tiny.They have tiny plates, tiny bowls, and tiny cakes and desserts.

Their Pizzas are one size smaller than the Western equivalents. That's just the way they do thing. You don't notice in anime as much about the scale of the food because you mentally assume their plates are the same sizes as yours.

I've seen morons on diets still consume 4000 calories a day. You could eat shit all day and still be skinny if you eat below your basic metabolic rate.

While that's thermodynamically correct nutrition is more nuanced than that.
Metabolic pathways are different for different nutrients. Glucose and starch mostly go to glycogen in the liver, fat obviously doesn't while fructose shares its pathway with alcohol breakdown. Not sure how proteins are handled.

Not only does the efficiency of those pathways differ but also the responses they trigger (insulin, appetite, fat uptake of adipose tissues etc.)

And depending on fiber content and complexity of the carbohydrates you might simply end up shitting out some of the calories before they get digested.
