It's the manifestation of Guts' anguish and anger towards everything that has happened in his life, it isn't really a separate entity but rather the pent up violence he has in his conscience; he was always violent, mind you, but the Hellhound became what it is because Guts kept supressing those feelings. Also, the Berserker Armor could have given the Hellhound its "shape", since the armor brings out your feral instincts.
