The guys who worked on Gurren Lagann and PSG either left with Anno when he created Khara a few years ago, or more recently left with Imaishi to create Trigger.
Apparently Gainax did something to piss Anno and Imaishi off, and they took all their friends with them when they left. I've no clue on the actual details though.

That doesn't explain why they didn't do anything to follow up their big hit (TTGL). PSG sold terribly in comparison, and yet it's still their best attempt since TTGL.

PSG was their attempt to follow up on TTGL. I suppose we could also count Rebuild of Evangelion 1.0, since at the time Rebuild was to be a joint Gainax-Khara production... until Anno decided to pursue full ownership of Evangelion, fought Gainax for the rights, and won. Before that happened I'm sure they were counting on revenue from Rebuild that stopped after 2.0 was released.
Did Medaka really deserve such low sales? Or was it some marketing fuckup?
Anno founded Khara in 2006 to pursue making Rebuild of Eva, but didn't officially leave Gainax until late '07. He didn't leave on bad terms at all. He hired Gainax to help do background and in-between work on the first two Rebuild movies.
I don't know Imaishi's full reasons for leaving Gainax, but I doubt he burned his bridges. He definitely kept in touch with Anno, since he has Khara helping out with the animation on Kill la Kill now.

Gainax is just a dump studio now, like DEEN or GONZO. They have no creative control, it's just a manga or LN production committee dumping them with a project. Though, the more they generate low sales, I'm sure those production committees would almost rather go to JCStaff or Madhouse.
C3 was shit but i still liked it. It was better than most of the shit airing that season.
But Anno did cut off that cooperation with Gainax after the second Rebuild, and wants the full rights to Evangelion now, included what he directed at Gainax. While Rebuild started as a partnership, Anno has had issues with Gainax in the past; that's part of the reason from ~2003-2006 he pursued a not so successful career with live action movies. I think the disagreement stemmed from problems with a Gainax adaptation of His and Hers Circumstances- the original mangaka didn't like the direction that Anno was going and asked Gainax to take him off the project, and they did. Anno wasn't too pleased.

Theyre more fucked than before, theyre shit sells for peanuts and they no longer have anyone with a modicum of talent since they all left for Khara or Trigger so the chance for Gainax to come up with a life saving project ala Evangelion or TTGL is very close to zero, I'd be genuinely surprised if they did.
I don't think BD of NGE can be done until it's settled who has the rights to the original. And I think that is still up to debate:

Medaka was literally a slideshow of the manga, it was barely fucking animated.
It's all right, but becomes a bit navel gazing torwards the end.
Not much QUALITY, didn't cut much material, had a good soundtrack, and the VAs did a good job. It was a fine adaptation.
I have some hope, since it's usually in that situation when people usually say 'fuck it' and go for one last hurrah. But the question is if they even got the talent or a project to even do that, plus there's the whole issue of luck.

The owners of LN pay the studio to animate their LN in order to advertise it to boost sales.

Yes, its really good and quite possibly the best thing Anno has done besides Eva. the original manga is so-so typical shoujo with lots of drama and people fucking people who they should not be fucking, but Anno managed to a whole other perspective and talk about growing up, and the human psyche and shit.
Even the low budget parts are nice. Yes, they are literally the manga stills with voice overs, but the VA cast is amazing and it really shows.
The only bad part about it is that there's no ending. The mangaka basically got jelous and told them to stop. so it ends literally in the middle of everything.
And reading the rest of the manga just to have conclusion is painful as fuck.
When a studio adapts work from an LN or manga the studio generally get paid a fixed fee by the owners of the IP to cover the budget and make a bit of money aswell. Like if you invited builders to make you a conservatory, you would ask them all how much theyd do it for and pick which one looked the cheapest/best value for money. What happens with the discs is largely irrelevant to studios most of the time unless they are on the production committees.
Gainax sucks because all their talent left for Gonzo, Trigger, and Khara.
Also the OST is great aswell, that little violin bit is fantastic. It is just so cinematic in places just like Anno really.

It's pretty normal to run a business as at a deficit for a while, especially if they have a track history of success like Gainax does. As long as someone is willing to extend them credit, why wouldn't they do it? The individuals involved have no personal liability for the debt accrued.
If there were shareholders of Gainax, I wouldn't want to be one of them.
