How can mirrors be real if our eyes aren't real?
Well, reflections are a bitch to draw. Couple that with the tight production schedules most TV anime face and you're bound to get mistakes.
but generally speaking, and in the above example (as in similar to it) it'd be both easier and more wise to just don't make a reflection at all. It doesn't need to in there in the first place, but since it's there but fucked up, it only draws attention.
It'd also draw attention if it wasn't there, "IS SHE A VAMPIRE???"

Reflections are hard as fuck to draw because our brains can't imagine them properly, this isn't an "anime only" thing.

Then get some reference shots for different poses next to mirrors. That gives you the idea of what you should be drawing.
And really, that's an excuse either way. All you need to do is recognize that it's an easy place to make a mistake so you make your grey matter work a little harder to realize when you are doing something wrong.
