Exodia's supposed to be rare as fuck. Apparently, almost no one's ever legitimately managed to collect the full set, and not only that, the few who were have never managed to summon all five in one game until Yugi did.
Funnily enough, even though the fully completed Exodia's effect of automatically makes it the (then) most powerful monster in the game, it's still not nearly as important as stuff like the Blue Eyes White Dragon or the God Cards for some reason. It has no notable history behind it; it's just a set of five cards with a unique effect.

Also 'fake cards' dont have monsters spirits in them. If you make fake cards of the more powerful monsters, like the Gods, the spirits will come and kill you for disrespecting them like that. Or if you do things like mark/edge your cards to cheat, the spirits might turn on you and seek revenge (Pandora's Black Magician)

Twice more as cards in the original and a few times in GX. The spirit appears in the final arc of the original.
A random Ghoul used it in Battle City and Yugi defeated him
Gozaburo Kaiba used an Exodia Necros deck.
Yubel-controled Martin used it against Amon in GX

People design cards, are produced by Industrial Illusions/whoever, and then those cards gain souls. Alternatively, some magic thing already exists and then manifests itself in the form of a card.
Fake cards are counterfeit cards. That's about it, game systems and the like recognize them as legal but the weird magic bullshit sometimes thinks it's bullshit to pull that off and does dumb shit, it's doesn't make any fucking sense really.
Egyptians aren't the only designers of cards, we see a number of people make up cards in GX and those cards have duel spirits, so the magic is super weird and works whenever it wants to.

Yeah it's definitely in your hand, I had an exodia deck or two in the videogames (never actually played Yugioh IRL.)
It sort of makes sense because if they're not all in your hand the pieces are not "together". But more importantly it makes it a bit more difficult to win (can't send the cards from the graveyard to the field, the opponent can force you to play monsters etc.)
During the battle city arc, that one guy Yugi dueled had 3 of each piece, so I guess there are more out there.

I like how even villains not connected to the Egyptians still have artefact duel disks and are super picky about playing Duel Monsters. It made sense during the initial series: The cards were actually magic and related to their ancestry, but Dartz actually predates the egyptians.

GX is better and 5Ds is better than that. Zexal is pure opinion. I liked it from the point it stopped being gay up until everyone started dropping dead left and right. So from episode 70-120 is the real "meat" of the show.
