I can understand reacting to a bullet to a degree but once it's in him he's have to mess with his internal organs which by and large is not a good way to continue to live.

Doesn't really work that way. It doesn't matter if Accelerator thinks something is dangerous or not, all that matters is how he has his reflection programmed. For example, things that Accelerator thinks are dangerous can still get through if it's not programmed in or if it gets in through a loophole (like Dark Matter did). He knows it's dangerous but his reflection doesn't know until he updates it (again, like against Dark Matter).

Except not really, because he doesn't use his reflection that way, he's not constantly adjusting it based on his own preconceptions. He sets up rules well ahead of time, and it follows those regardless of whether Accelerator thinks something is dangerous or not. He has to take time to change the laws when he wants to adjust it for whatever reason.

It can tell amounts. It doesn't reflect the force from picking something up so he can pick up things fine, the same with walking, but it'll reflect the force of a bullet because that's harmful. It'll also make sure he doesn't burn up (it protected him from lava), but it won't chill him to death.

Easier way to think about it is the reflection reflecting anything "harmful". It's all reality warping anyways, and it's not the only conceptual Esper power. Measure Heart can change the distance between two people's hearts for example (in that she can make it so you feel the same about her as you do your girlfriend, so you won't be able to kill her and she can walk up and slit your throat).

If you're talking about the "Don't Reflect" list, yes that was me. Did I make it seem more flexible than it is?

He doesn't change the list on the fly (well, he did against Dark Matter but that took special effort and he doesn't do it often), he just lets it do it's thing. If he sees something he wants to eat, he doesn't add it to the "Don't reflect" list. If it's safe it's already there, if it's not it bounces.

Now he could add something dangerous to the "Don't Reflect" list if he wanted. Like if he added mercury to it for some reason, even if he knew it would kill him and was dangerous, he could still drink it.

It's all pre-set basically.

Some poisons would be missed and some typically harmless things could be used to murder him.

Unless he adds poison to the list for some reason I don't see why it would work on him. And we already know his power can tell amounts and not to give him too much of something.

Now circumventing what his reflection deems as "harmful" is a very good idea, Kakine did it. His power is to create matter which doesn't follow the laws of physics. He filtered sunlight through it, which gave it properties impossible in the Toaru universe. It became deadly but Reflection still saw it as sunlight and let some through. Accelerator had to figure out what laws of physics Dark Matter was based on and add it to his reflection in order to start blocking it.
